We want to make working with PrimeCare as easy as possible - this includes accurate and complete data for each trip. Below are several ways to get your trip on our Schedule.

1) Use our Online Trip Request Form

This will only take a few mins and a credit card to secure your trip.

Then FAX it to

5) Send us an EMAIL with answers to all the Required Trip Details listed below -

  • Is this a Round-Trip or One-Way? (If One-Way, is this a Discharge?)

  • Type of Service - Ambulatory, Wheelchair, Stretcher

  • (if Wheelchair, do we need to bring one?)

  • Patient/Rider First & Last Name

  • Patient/Rider Date Of Birth

  • Patient/Rider approx Weight

  • Will they have an escort riding along (we allow 1 to ride for free)

  • Name of person placing the order

  • Person responsible for payment
